Some notices are provided here for your review before beginning therapy. Client-specific forms, intake documents, and consent forms are completed securely through the client portal as part of the intake process.

If you have questions about any notice, document, or policy, we can discuss them during the consultation or early phase of therapy.

Good Faith Estimate / No Surprises Act Notice

Under federal law, clients who are uninsured or not using insurance have the right to receive a Good Faith Estimate explaining the expected cost of services.

This notice is provided so you can better understand your rights and ask questions about the cost of care.

Notice of Privacy Practices

Your privacy is important. The Notice of Privacy Practices explains how health information may be used, disclosed, and protected as part of therapy services.

California Required Notice to Clients

California clients may review the required notice regarding services provided by a California Licensed Marriage and Family Therapist and information about contacting the California Board of Behavioral Sciences.

Insurance, Superbills, and Out-of-Network Benefits

Soulful Relationship Therapy may provide services on a private-pay, out-of-network, or insurance-billed basis when applicable.

If you have out-of-network benefits, I can provide a superbill that you may submit to your insurance company for possible reimbursement. Reimbursement depends on your specific plan, deductible, and coverage.

It may be helpful to contact your insurance company directly to ask about out-of-network mental health benefits, reimbursement rates, deductibles, and any requirements for submitting claims.

We can discuss payment, insurance, superbills, and any questions about fit during the initial consultation.
